By signing below, I acknowledge agreement with the following TERMS OF REGISTRATION:

We, the parent(s)/guardian(s) of the above registered participant in the NKGSL Softball, hereby give my/our approval to participate in any and all activities, including transportation to and from the activities. I/We know that participation in softball may result in serious injuries and protective equipment does not prevent all injuries to players, and do hereby waive, release, absolve, indemnify and agree to hold harmless the New Kent Girls Softball League, it's officers and directors, any organizers, sponsors, for any claim arising out of any injury to my/our child whether the result of negligence for any other cause, except to the extent and in the amount covered by accident or liability insurance.

I/We, the parent(s)/guardian(s) of the above registered participant, agree that youth sports is solely for the education and recreation for our children and I/we will not participate in, nor tolerate, unsportsmanlike behavior on the part of myself, other parents or staff. I/We understand that by signing this agreement, and upon payment of registration fees, that I/we become a member of the NKGSL and are responsible for the actions and appearances that the NKGSL presents during all its activities. I/We also give my/our approval to use photographs and or images of my/our child on any promotional publication materials (including the NKGSL website).

• NKGSL requires all players to participate in fund-raising efforts as suggested by the Director of Fund Raising.

• NKGSL requires all players to provide volunteers to work in the Concession Stand as coordinated by your team.

• NKGSL Players, Coaches, Parents/Guardians must maintain an ethical code of conduct when attending any function given by or related to NKGSL.

• No refunds after the uniform order has been placed for any reason.
