2009-2010 AVHS Athletic Packet Procedures (& Individual Forms)

1. READ the Student Handbook for Interscholastic Athletics and Co-Curricular Activities.
2. TURN IN a completed and signed Athletic Participation Agreement Form. (Form 1
3. TURN IN a completed and signed Interscholastic Athletics/Co-Curricular Activities Code of Conduct Form. (Form 2)
4. TURN IN a completed and signed Athletics Eligibility Screening Form. (Form 3)
5. TURN IN completed and signed NCS Non-Use of Steroid and Ejection Policy Form. (Form 4)
6. TURN IN a completed and signed Athletic Insurance Waiver Form. (Form 5)
7. TURN IN (3 COPIES) completed and signed Tri-Valley Orthopedic Athletic Training Contract and Emergency Information. (PLEASE PRINT 3 COPIES 1 for athletic trainer, 1 for office, 1 for coach) (Form 6)
8. COMPLETE Sports Physical questionnaire and bring with you to your physical. This form STAYS with the doctor. (Sports Quest Form)
9. TURN IN a current Physical Examination Form signed by a doctor. An annual examination, or a statement by a medical doctor certifying that the student is physically fit to participate in athletics is required every year. Please note, chiropractor exams are no longer acceptable. (Physicial Exam Form)
10. PAY the athletic fees (varies depending on the sport )at the Activities office or online at the Webstore or by following this link: http://webstores.activenetwork.com/school-software/amador_valley_high_s/ (If paying online, BRING YOUR RECEIPT with you as proof of payment.)
11. PURCHASE an ASB Student Body Sticker ($65.00) at the Activities office or online at the Webstore by following this link:

12. CLEAR any debts or book bills you may have with the school at the Activities Office.
13. CHECK that you are academically eligible to play a sport. Grades from your most recent grading period must reflect a 2.0 G.P.A. with no "F's." Grades will continue to be monitored throughout the season by the Athletic Director.
14. ATTEND mandatory sports meeting.

DO NOT GIVE THESE ITEMS TO THE COACH
ALL ITEMS SHOULD BE RETURNED TO THE STUDENT ACTIVITIES OFFICE WINDOW.

The student Activities Office will be open starting August 3, 2009 from 7:30 a.m. to 3:30 p.m. No Athlete will be allowed to practice or participate in any game, meet, match, etc., until all pertinent documents have been turned in to the Student Activities Office. A clearance receipt will be issued verifying the completion of the process.
