4/23/2009Athletic Requirements

All participants need to have a A.I.A. (Arizona Interscholastic Association) physical on file with the Athletic Director's office and signed by a medical doctor.

All participants must have passed all classes from the previous quarter, or made up the same number of failed classes in summer school. (Summer school classes do not have to be the same as those failed).

All participants must pay a participation fee prior to issuance of game equipment. This fee is $200 made payable to the Horizon Athletic Department.

One of the participants' parents and each athlete must haqve seen the Informed Consent presentation before tryouts begin and have signed the drug testing release forms. This video needs to be seen once in a student's 4 years, but new paperwork must be turned in annually.

Informed consent dates TBD at 6:00 pm in the Auditorium.

NOTE: Students who transfer may not be eligible to compete in athletics upon entering Horizon. Please call the Athletic Department at 602-44-6023 if you have questions about athletic eligibility.

To be eligible for sports, as well as any extra or co-curricular activity which involves competition follow this rule: "NO PASS - NO PLAY". All students must take and pass a minimum of four (4) classes each nine weeks.

An athlete must remain academically eligible. In addition, parents and athletes must read the "Parent and Athlete Informed Consent, Codes of Conduct, and Random Drug Testing Handbook" each year.

Parents/Guardians must ateend a parental consent meeting (food for four years) to become acquainted with the policies and procedures regarding sports participation. Times and dates are given in school announcements and are posted on our calendar of events/schedule.

Spring Football Begins for all sophomores, juniors and seniors on May 4, 2009 at 2:30 pm.
5/7/2009AIA Fee
All players must pay the AIA fee prior to the first football game. The fee is $200 and is payable to Horizon High School. The maximum for a family during a school year is $400. You may use your tax credit when paying this fee. The tax credit amount is $200 for taxpayers who file as a single and $400 for taxpayers who file jointly. Please turn in a copy of your birth certificate with your physical form to your Head Football Coach or to the Athletic Office.
5/7/2009Consent Video and Release Forms
To participate in any sport, one of the participants' parents and each athlete must watch the informed consent video before the season begins and each player must have a signed drug testing release form. This video needs to be seen once in a student's four (4) years, but new paperwork must be turned in annually. The informed consent video dates are (TBD). Please note, if the player has not seen the video prior to the start of the first official football practice (August 10th), he will not be allowed to participate until the presentation has been seen.

5/7/2009Participation Fee / Husky Gridiron Boosters
The participation fee for Husky Football is $125 payable to Husky Gridiron Booster Club. This fee can be mailed to 9290 E Thompson Peak Parkway, Lot 450, Scottsdale, AZ  85255 and/or you can give to any one of the Head Football Coaches. These fees are used to help cover some of the expenses for Horizon Football that is not covered by the district. You cannot use your tax credit money for this fee, only for the AIA fees, as stated under AIA fees. The Parent Booster Club conducts many opportunities to raise funds through throughout the season in an effort to support our teams and keep our program going.
