To Register to play football the parent and player must see the consent film and sign the consent form. To play football the player must also take a physical and have the Medical Release completed on the Physical Form every year as well. See Physical and Consent Form links below. The Physical form comes from a link to the Horizon Athletic department website. All incoming freshmen must attend with a parent/guardian (date TBD) at 6:00 pm in the Auditorium.
