In consideration of your acceptance of me or my child as a participant in the Edina recreations program, I hereby waive all claims against the City of Edina, its employees and agents, from all claims for injuries suffered by me or my child incidental to, connected with, or arising out of the recreational activities for which I am or my child is enrolled, including injuries suffered as a result of negligence by the City of Edina or its employees or agents but not including injuries suffered as a result of their willful or intentional misconduct or gross negligence.

I do hereby waive, release, absolve, indemnify and agree to hold harmless the Edina Girls Fastpitch Association (EGFA), its Board of Directors, members, organizers, sponsors, coaches, supervisors, participants and persons transporting my child to or from activities, from and against any claims of injury or harm to my child incidental to, connected with or arising out of EGFA activities.

I understand that the program described on the reverse side for which I have enrolled or for which I have given my child's permission, may be hazardous and that injuries may occur in the normal course of participation or instruction and I assume all risks and hazards incidental to me or my child's participation including transportation to and from activities.

I understand and acknowledge that EGFA and the City of Edina have no medical or health insurance covering me or my child. I agree to return upon request the equipment issued to me or my child in as good condition as when received, except for normal wear and tear.

I certify that to the best of my knowledge, I have or my child has no physical injury or sickness except as specified above. Clicking through this document to complete registration electronically binds me to the terms specified with the same force and commitment as if I had signed the document manually.
