This form can be filled out online and printed or printed blank to be filled out by hand.

Risk Management Disclosure Form
Must be turned in for all administrators every year for a card to be issued.

Minor Adninistrator Form
Here is the form that must be completed and turned into Pat along with a
copy of the administrator membership form for administrators under the
age of 18. Cleared administrators under 18 years of age are required to
be under the direct supervision of a risk management cleared
administrator over the age of 18 years.
