Registration - Winter 2009-2010

Once you submit your registration you will have the option of paying by credit card or check. If you choose to pay via credit card simply follow the directions on the PayPal site. You do not have to establish a PayPal account in order to pay via credit card.

Credit card payment is preferred.

You may use one check for multiple players, however you must include the printed registration confirmation for each player. Payment cannot be applied to your child's registration without including this printed information.

Players will be prioritized for capacity and planning issues based on when the registration fee payment is received, not when the registration form is submitted. Credit card payments are applied immediately.

CYM rules prohibit individuals in 9th grade through 12th grade from participating on a CYM parish team and any high school team (Freshman, JV, Varsity, etc.) in the same sport.

I hereby give my consent for the above named individual to participate in CYM athletics during the current program year. I recognize that there are certain risks of physical injury in playing competitive athletics. Excluding intentional, deliberately inflicted, and illegally caused injuries, I further agree, in consideration with CYM's sponsorship of beneficial athletic competiton, to release the office of the CYM of the Diocese of Wilmington, the Catholic Diocese itself, the St. Mary Magdalen Parish, and all of their employees, directors, administrators, coaches, and staff from all legal liabilities for accidental injuries suffered by my child as a result of participation in athletic activities, or travel to and from any athletic event. Providing, however, that recourse is reserved to seek damages, medical and hospital expenses, and court costs for any such accidental injuries to my child incurred during a scheduled event from any liability insurance carrier within the limits of its liability policy. I affirm that the information above is true and correct. I agree to complete the online CYM registration, providing additional medical information as required.

The SMMAA is not responsible for changes to a player's team assignment, or any other changes or modifications that must take place, in response to inaccurate or incomplete information supplied by the individual completing this registration form.

By initialing and checking the box below you are accepting the terms of all listed above and you are also confirming that you have reviewed all of the materials in the "SMM Policies" section of the website and that all players and family members, including parents, are agreeing to comply with any and all said policies at all times. The application of said Code of Conduct is not exclusive to SMMAA and/or CYM sponsored events and the SMMAA reserves the right to review any incident that involves a player, parent, or family member who is an active member of an SMM sports team and to take action as deemed appropriate after a review conducted by the SMMAA Board.
