TO REGISTER:

Click on the link below to download the registration paperwork. Print and complete the registration package. Gather the following:

• Current Photo of child (to be turned in with Registration Paperwork - including returning players)
• Copy of Childs Birth Certificate (including returning players)
• Completed Physical** date April 15th, 2013 or later

Once all paperwork is complete and gathered, mail it along with payment*** to:
