8th grade parent night - FALL ATHLETIC MEMO

 

NOTE: FROSH AND JUNIOR PHYSICALS HAVE TO BE DATED AFTER MAY 1, 2015 TO BE ACCEPTED FOR THEIR 2015-16 FROSH/JUNIOR YEAR.

 

FREE SPORTS PHYSICAL @ The Center – donations accepted.

BOYS – August 6, 2015 ~ 5:30pm

GIRLS – August 11, 2015 ~ 5:30pm

NOTE: All incoming freshman and juniors MUST have new sports physicals per OSAA rules and regulations.  If you did not get a sports physical as a freshman or junior then you must get one as a sophomore or senior. WE will only accept the OSAA sports physical examination form which can be found on the OSAA website.  Please go to www.osaa.org then Governance tab, forms and scroll down to physical examination form (Physical Examination
version last updated May 2010) or you may pick one up from our athletic office.

 

FALL SPORTS / CLUBS

 (Football, Soccer, Volleyball, Cross Country, Water Polo, Dance & Cheer

 (Bowling & Archery -no physical needed)

 

The Athletic office will be open Aug. 3rd – 14th. Hours may vary due to meetings. Also athletes may pick up insurance forms. This is a separate policy and is affordable. We highly encourage you to look into this.

 

* NIGHT HOURS * the accounting office and our office we will be open extended hours Aug. 12th from 4-7:30pm.  Please come in and clear. WE will have a laptop to use if you need it for on-line registration.

  1. August 14th - All athletes must be cleared by the end of this day.
  2. The Athletic office will be closed August 17, which is the first mandatory day of practice. Bowling & Archery practice starts later in Aug./Sept.

In order for athletes to be cleared for their declared sport –

  • All athletes must have passed 5 of 7 classes and maintained a 2.0 GPA the last nine weeks of school (June – 2015 report card)
  • All athletes must be on track to graduate
  • All athletes must be cleared off the FINES list.
  • First sign up on-line.
  • Second pay in the accounting office.
  • Third – between Aug. 3 - Aug. 14 please bring paid receipt & sports physical if applicable to athletic office for initial clearing. If everything is in order your will receive a yellow clearance slip. You must have this slip to practice.

Pay to play fees - $100.00 per sport. Club fees may vary.

You can help us earn even more in 2015!

 

 

With the return of Community Rewards, Fred Meyer is donating $2.6 million this year to non-profits in

Alaska, Idaho, Oregon and Washington, based on where their customers tell them to give.

If you have already linked your card, thank you! If not, here’s how the program works:

  • Sign up for the Community Rewards program by linking your Fred Meyer Rewards Card to (Bend High Softball) at www.fredmeyer.com/communityrewards. You can search for us by our name or by our non-profit number (insert NPO# here).

  • Then, every time you shop and use your Rewards Card, you are helping (non-profit) earn a donation!
  •  
  • You still earn your Rewards Points, Fuel Points, and Rebates, just as you do today.

  • If you do not have a Rewards Card, they are available at the Customer Service desk of any Fred Meyer store.

2015 Spring Clearing Memo

 

 

Spring clearing begins February 9. All athletes must be cleared by Feb. 27 to be eligible for the first mandatory practice, which is March 2.

Initial clearing process for declared sport / club:

 

1. Must create an account and REGISTER @ https://www.familyid.com/bend-high-school.

 

2. Must be cleared off the fees & fines list.

 

3. Must pay the pay to play fee at accounting office or on the Bend-La Pine School website. RECEIPTS WITH A RED MARK WILL NOT BE ACCEPTED.

 

4. Have a current sports physical.

ALL OF THE ABOVE MUST BE DONE BY Feb. 27 TO PRACTICE/PARTICIPATE at the FIRST MANDATORY PRACTICE MONDAY March 2.

 

Attendance and Release From Class • In order to participate in practice or contests, the participant must be in school attendance the full day of such practice or contest. • Each athlete must attend school up to the time of dismissal. • Athletes will not be removed from class for other than scheduled contests.

NOTE: No school Feb. 16 – Presidents’ Day
