My child’s participation is voluntary and I voluntarily permit my child to participate. I understand that participation in QUAKERTOWN GIRLS LACROSSE, also known as QTOWN GIRLS LAX, is an inherently dangerous activity, and that the risk of participation include, but are not limited to: falls, collisions, cuts, broken bones, severe injury and even death. I hereby, for myself, my child, my heirs, administrators, executors, personal representatives and assigns, forever waive, release and discharge any and all rights and claims for damages and losses, whether monetary or otherwise compensatory, that I or my child may have against: (i) QUAKERTOWN GIRLS LACROSSE and its directors; (ii) QUAKERTOWN GIRLS LACROSSE executive directors, owners, managers, officers, employees, members, representatives and agents; (iii) all coaches, participants, organizers, supervisors, planners and volunteers; and (iv) all city, county and state governments and school district and administrative organizations, including any participating booster organization(s), for any and all injuries, property loss or damages, sustained by me or my child arising out of association with, entry in, or participation with the QUAKERTOWN GIRLS LACROSSE activities. I understand and agree that medical or other services rendered to my child by or at the insistence of any of the above parties are not an admission of liability to provide or continue to provide any such services and is not a waiver by any said parties of any hereunder. I also acknowledge that should my child require transport to a medical facility, I must pay for such transportation and any treatment period. I further agree now and forever to hold the above named and unnamed parties harmless and indemnify them for all claims, damages, judgments and costs of whatever nature and form. QUAKERTOWN GIRLS LACROSSE recommends that your child be examined by his physician before participating in any QUAKERTOWN GIRLS LACROSSE games, clinics or events. If my child has a history of heart diseases or brain related health issues, she will consult a physician prior to participating in any QUAKERTOWN GIRLS LACROSSE games, clinics or events.
Before signing my name to this Release, I state that: 1) I have read it; 2) I understand it & know that I am giving up important rights; 3) I sign it freely as my own act and deed; 4) I intend to be legally bound to it; and 5) I certify that my child has been examined by a certified physician and is physically fit to enroll in the activity mentioned above.
Playing time – this is a competitive travel team, if a player misses a practice(s)/game(s), it may affect the players amount of playing time in games.
Positions – Coaches have final say on the position of players, by determining what is best for the team and the player.
Feedback – feedback from parents and players is welcome, but Coaches have the final say on playing time, positions and practice format.
Registration $100 for 3rd-6th grade players. Sister Rate - $60 for 2nd player and $45 for 3rd player from the same family.
$50 for 1st & 2nd grade players.
