The school has transitioned to DIGITAL paperwork. There will be no more handing in forms physically. There are no exceptions and this must be done to clear your child before they can participate in our program(s).

- Video certificates (listed below)
- Proof of insurance (student's name must be on the card)
- Copy of birth certificate
- Parental authorization form (Parental_authorization.pdf)
- Athletic Creed (Athletic Creed.pdf)
The athlete has to complete these three vidoes, download the certificates and include them when uploading the other forms. Click on the links provided, make a profile/account, watch the videos, download the certificate at the end, and upload it to their athletic clearance profile:
