Athletic Paperwork

All Student-Athletes must complete the following items to be Cleared for any Extra Curricular Activity (workouts, tryouts, practices, & games)

 

  1. Current Medical Physical on the FHSAA EL 2 Form (click) - MUST print, fill out, Dr signature, good for 1 calendar year, Upload as 1 document
  2. Must Purchase School Insurance (click) - MUST purchase insurance & print out Insurance Card
  3. Must complete & earn 3 FHSAA Video Certificates -  Print out each certificate of completion & upload as 1 document
  4. Students Must Complete Registration Process on ATHLETIC CLEARANCE (click) & UPLOAD the proper Documents (#1-3)

ALL DOCUMENTS MUST BE UPLOADED AS A PDF not JPEG (No Pictures)

 

 

TRANSFER STUDENTS

Any student who has attended another high school before enrolling at Seminole High MUST fill out the GA4 (click) and turn into Josh Walker at Seminole High School

 

 

NON-TRADITIONAL STUDENTS

Any student that does not attend Seminole High registered as a:

  • Home Education Student
  • Florida Virtual School Student
  • Attends Another School that does NOT have Sports (ex:  Jacobson High)

Must first complete the Following Paperwork to be considered and turned in Josh Walker at Seminole High School 

     1. EL 7 Form (click) - Home Education Participation

     2. EL 7V Form (click) - Home Education Verification

     3. EL 14 (click) - Student Controlled Open Enrollment (if applicable)

     4. Pinellas County Co Enrollment Application (click) - Must be Processed by Pinellas County Schools

            - This document must first be processed by Pinellas County Schools and then turned into Seminole High School

     5. GA 4 Form (click) - Recruiting Policy Affidavit
