GRAD NITE 2020 PERMISSION TO DISPENSE MEDICATION
Any student requiring medication during the Grad Nite time (from 10:00 pm on Thursday, June 4, 2020 to approximately 4:00 am on Friday June 5, 2020) must notify us and submit this form before the event.
For prescription medication: We require the medicine to have a pharmacy label on it indicating the student’s name, name of medication, dose and frequency to be taken. All medications must be current. A note from the student’s physician will suffice if the medication is well labeled and clearly identified. This includes EpiPens, pills, capsules, liquids and nasal or oral sprays.
For over-the-counter medication: If the student intends to bring analgesics (pain relief medications), decongestants, nasal sprays or any other common medications, a note from a parent is required stating that the medication may be needed. These medications will be administered in the event of medical necessity only and at the discretion of the on-site team member.
Any student found taking or carrying on his/her person anything that could be construed as a drug or medication, without a permission form on file, will be required to leave the facility. The parent will be called to pick up the student and he/she will remain with an on-site team member until the parent arrives.
