REGISTRATION FORMS - SUMMER 2025 BASKETBALL SESSION

 

The following forms 1 & 2 must be provided for all players who did not participate in the 2025 (spring or last year sessions) Davis Hot Shots program in order to be able to participate for Davis Hot Shots 2025 summer session.

 

Please fill out, sign and bring to registration.

 

1)   PLAYER_INSURANCE_FORM1.pdf  (new players to Davis Hot Shots only)

 

2)  PLAYER-PARENT_CONTRACT.pdf    (new players to Davis Hot Shots only)

 

3)  ALL PLAYERS:   Current AAU Card with Exp Date 8/31/2025 or later  https://aausports.org 

    (Click on Join AAU top left corner)
