All players must be current members of U.S. Lacrosse. Proof of current membership is required and should be submitted at the time of registration.
All players must submit a completed sports physical examination form each year, signed by a physician, stating that the player has been examined and is fit to participate in lacrosse. Sports physicals can be obtained at your local physician's office, Walgreens, CVS, etc.
All registration forms, U.S. Lacrosse proof, and medical examination form must be submitted PRIOR TO the player participating in team practice or games.
