Required Registration Forms for Fall Hockey:

(per AHAI, player cannot practice until ALL these forms are filld out and returned)

 

• Fall Hockey Waiver (Different from the Summer Skate Waiver) - (e-sign)
• Credit Authorization Form (if paying by Credit Card/Checking Account)
• Parent/Player Agreement - Email to Btheis@oswegohockeyclub.com
• IL West Zero Tolerance Policy - Email to Btheis@oswegohockeyclub.com
• IHSA Physical form from your Doctor (due by 10/1) - Email to Btheis@oswegohockeyclub.com
• Safesport Training (required for birth years 2006 and 2007)

Please complete all the above ASAP so we can get rosters submitted to AHAI!

The process to file an offical complaint for Board review at our next monthly meeting. 

1. Reach out to the coach for clairification on the issue if it's hockey related.
2. Reach out to the Hockey Director if coach was unresponsive or you feel your complaint needs to be escalated.
3. Fill out the below form to bring your complaint to the Boards attention. This will become a discussion topic at our next monthy meeting (you might be asked to attend said meeting to help understand the situation)
