Both Medical Releases & Team Jot Forms must be complete to be considered checked in and eligible to play.  

Any player not completing a Medical Release Form will not be allowed to play.  

Any team not completing the Coaches/Team Jot Form will NOT be placed on the schedule and will be refunded.

Medical Release forms need to be completed by each player.

 The Jotform will be sent to the Tournament Director and to the parent completing it. 

Each coach is required to carry the medical release with them at all times.  

It must be made available for review by the tournament committee at all times upon request.
