All fees are NONREFUNDABLE and must be paid once form is submitted via cashapp or Zelle. You will not be able to tryout or receive address to tryouts unless payment is received. WAIVER OF LIABILITY I, acknowledge, agree and understand that: 1. Voluntary and of my own free will, elect to participate as a member of the Carolina Panthers Non-Profit Community Support Organization and Minor League Professional basketball team during the 2027 East Coast Basketball League Season. 2. There are certain risks and hazards involved in participating in basketball that may result in injury or death to other players and myself, including, but not limited to those hazards associated with weather conditions, equipment and other participants. 3. Basketball is a contact sport, and while fighting is strictly PROHIBITED, I understand that incidents can occur that are dangerous to others, including myself. 4. The very nature of the game of basketball is hazardous and risky, including, but not limited to, the acts of jumping, shooting, running, diving, stretching, sliding and collisions with other players and stationary objects, all of which can cause serious injury or death to other players, as well as myself. 5. There will be NO tolerance of verbal, or physical disrespect towards referees by Team, Coaches, Staff & Players, including myself. I acknowledge that I have thoroughly read and that I completely understand every one of the provisions is this player liability waiver.
