Important Forms (Required for All Players Each Season)

These forms are required for all players and need to be signed and submitted for each season.  If you participated in our fall season we cannot reuse your forms for the spring season.

 

MEDICAL RELEASE: Medical Release
 

PROOF OF RESIDENCY & AGE REQUIREMENTS: Proof of Residence and Age Requirements

 

SCHOOL ENROLLMENT FORM: School Enrollment Form

**School Enrollment Boundary Form must be signed by the Principal or Vice-Principal only and must be attached to a letter from the school on school letterhead stating enrollment**

SPONSORSHIP FORM: Sponsorship Form
(Email the company logo to info@bocalittleleague.com, the file needs to be 358 pixels wide and 72 pixels high formatted as either .jpg .png .tif)
