All players must complete the following form to participate in CTBL activities.

 

 

You do not need a paypal account to pay your waiver fee.  It is the payment processor the league uses.  Thanks for your understanding.

I hereby release the CTBL from any and all liability from injury or insult occurred while participating in league activities.

 

I acknowledge the CTBL is a private, recreational league. As such, I retain no right to participate in the league or league activities should my conduct on or off the field be viewed as dangerous, detrimental to the league or any other grounds deemed to make a player ineligible by the league executive.  i shall seek no refund of fees or waiver should I be disqualified form CTBL activities.
