1. I understand the risks and dangers inherent in participating in baseball games and events (''League Events'').

2. These risks arise from, but are not limited to, physical exertion, equipment use by me and others, physical contact with others, exposure to outdoor elements, exposure to infectious disease such as COVID-19. These risks can lead to illness, personal injury, death, and property damage.

3. I am assuming full responsibility for any risk of death, personal injury, illness, or property damage suffered by me while participating in League Events.

4. In exchange for participating in League Events, I voluntarily release St. Louis Baseball Forever League, Inc., its officers, agents, members, volunteers, and employees (collectively ''STLBFL'') from all liability resulting from or arising out of my participation in League Events, even if caused by the acts or omissions of STLBFL or others.

5. I agree to abide by the Code of Conduct as set forth in paragraph 10:02 of the STLBFL playing rules. I understand that if I am found guilty of violating the Code of Conduct, I shall be subject to the penalties set forth in the Table of Penalties chart. I further understand that violation of any of these rules can lead to probation, suspension or life time expulsion from the league.

6. By my manual or electronic signature below, I release, discharge, waive, and forever relinquish any actions or causes of action I may have or have had, whether past, present, or future, whether known or unknown, and whether anticipated or unanticipated by me, arising out of my participation in League Events. This Release constitutes a complete release, discharge and waiver of any actions or causes of action against STLBFL.

7. This Release will bind me, my spouse, my heirs, my personal representatives, and my assigns.

I HAVE CAREFULLY READ AND FULLY UNDERSTOOD All PROVISIONS OF THIS RELEASE, AND FREELY AND KNOWINGLY ASSUME THE RISK AND WAIVE MY RIGHTS CONCERNING LIABILITY FOR MY ILLNESS, PERSONAL INJURY, DEATH AND PROPERTY DAMAGE. I UNDERSTAND THAT BY TYPING MY BELOW, I AM SIGNING THIS DOCUMENT USING AN ELECTRONIC SIGNATURE, WHICH IS JUST AS LEAGALLY ENFORCEABLE AS IF I HAD SIGNED MY NAME TO A PAPER COPY OF THIS DOCUMENT.
